There’s no form. You send a copy of the report card in with a written request. If it’s allowed in your state, and Mercury offers it, they’ll give it to you.

You need to have a school official complete & sign this form then send it to your insurance company.
The discount is for young drivers only if available. Also, a copy of the student’s transcript should work too.
